- date found
- 2026-10-22
- kind
- error
- where
- Section 8 and source [56] (v1.1 and v1.2)
- what (plain language)
- The 9 May 2026 Campland closure in Mission Bay was described as having no cause named. The County did name one: runoff from a fire that morning at the neighbouring Mission Bay Boat & Ski Club. The error came from a secondary news relay that had dropped the cause, which is the very pattern Section 8 describes.
- found by
- One (self-found, 22 Oct)
- status
- resolved 2026-10-22 in v1.3 ([56] now cites two outlets reporting the County's stated cause)
OS-E02
- date found
- 2026-10-26
- kind
- error
- where
- v1.3 cover page (status and date lines) and the v1.3 essay's front matter
- what (plain language)
- The cover still reads "Status v1.2, final and settled" and "Date 21 October 2026." Every page footer, the change log and the package README correctly say v1.3, correction edition, 22 Oct 2026. The content is v1.3; only the cover lines were not updated.

- what (plain language)
- The satellite imagery is EOX's Sentinel-2 cloudless 2016 mosaic (built from Copernicus Sentinel data of 2016 and 2017). Newer imagery under an open licence has not yet been sourced (wall W068). Changes on the ground since 2016 are not shown in the imagery; positions come from records and OpenStreetMap, not from the image.

- what (plain language)
- When the headlines peaked has not been measured; that is the planned signal study's question (OS-E06). The sentence should say "as the sewage crisis drew wide coverage," or be dated to a measured count.

Note (Red). OS-E08 to OS-E11 were the four scope items Red and Blue raised on draft 2 (20 Oct; RB-79 §6). The editor chose to keep draft 2 as made for the white paper; the four sentences carried through to v1.3 unchanged, so they belong here. The two Section 8 wording edits Red asked for on 21 Oct are in v1.3.
Kinds. Error: a number, label, date or citation that is wrong. Overreach: a claim that goes further than its evidence. Open check: work not yet done that could change a finding.
